Evidence Based Programs for Healthy Living
Wellness Classes
Classes are taught throughout the 16 county region to provide information on taking control of your health. While geared to those over age 60, anyone can attend. Classes are all evidence based which means they have been proven to work via rigorous university testing. Classes include Chronic Disease Self-Management, Diabetes Self-Management, Matter of Balance (a fall prevention class), Tai Chi for arthritis and balance, and Powerful Tools for Caregivers. NOTE: There may be a fee attached with the class.

Self-Management Programs
CDSMP (Chronic Disease Self-Management Program) helps individuals who either have a chronic condition or know someone who does, to take control of their health through self-management, problem solving, and action planning.
DSMP (Diabetes Self-Management Program) focuses exclusively on people with Diabetes or their caregiver.
Workshop Duration: 6 weeks; 1 session per week; 2.5 hours per session.

Tai Chi
for Arthritis
Tai Chi is an ancient Chinese martial art known for its slow, fluid, and meditative movements. In this program, participants learn Sun style Tai Chi movement sequences that are meant to improve balance, breathing, and physical strength.

Powerful Tools for Caregivers
Do you provide care for a loved one? Powerful Tools for Caregivers is a program that equips you as a family caregiver with tools and strategies to better handle the unique caregiver challenges that you may face.
Workshop Duration: 6 weeks; 1 session per week; 90 minutes per session.
